To ensure the safety of our students + staff, the Lake Washington School District requires that every volunteer be screened and cleared by the Washington State Patrol.  Every two years you will need complete a volunteer application.

The fastest way to have your application processed is to complete the online form through Parent Access. You will need to have an electronic copy of your drivers license available to upload to the application in order to complete this process. If you do not have a scanner, you can take a digital photo of your license and upload that copy. If you cannot get an electronic copy of your license, you can send a copy of your license to the volunteer office at:

If you have never used Parent Access before, you will need to get a password.  Go to the Parent Access New User Page on the District Website for more information on how to sign up.

(You will need to have the latest version of Adobe Reader installed on your computer in order to open this document.) You can print out this application, complete it (please be sure to complete all of sections C & D on the Washington State Patrol form – a thumbprint is not required), then mail it, along with a copy of your driver license to the volunteer office at:
